Acts is currently seeking qualified candidates for a PRN Nurse Practitioner to join our expanding Primary Care Services and Nurse Practitioner Program. This NP position provides as needed coverage for our Carolina and Georgia community NPs at Matthews Glen (Matthews, NC), Tryon Estates (Columbus, NC), Park Point Village (Rock Hill, SC), and Lanier Village (Gainesville, GA).

 

In this role, you will be responsible for providing onsite, telemedicine and/or call coverage as needed, typically in one-week increments. Our NPs provide primary care and urgent care for residents across the continuum in independent and assisted living as well as skilled nursing. This will involve managing the residents’ health care needs in compliance with state and federal regulations as well as maintaining knowledge of the Long-Term Care Industry and geriatrics through continuing education.

Requirements

The ideal candidate will meet the following requirements:

  • Master’s degree in Advanced Nursing; preferably in geriatrics
  • Must be a licensed Nurse Practitioner with prescriptive authority in the applicable work state(s)
  • Minimum of five years' experience in an outpatient setting, geriatrics, and/or skilled nursing
  • Experience in managing palliative care and chronic care management services
