Acts Home Health is currently seeking qualified candidates for the role of Registered Nurse (RN) Case Manager for our Medicare Certified agency that serves two of our beautiful, gated communities in Media, PA.

In this role, you will be responsible for the coordination and supervision of all resident care provided within the agency.  Utilizes leadership skills to coordinate resident care services, serves as resident advocate, facilitates and ensures communication/collaboration among all agency team members.


The ideal candidate will meet the following requirements:

  • Current State license as Registered Nurse (RN)
  • Strong clinical assessment and case management skills
