Acts Home Health is currently seeking qualified candidates for the role of Registered Nurse (RN) Case Manager Per-Diem for our Medicare Certified agency that serves our beautiful, gated communities in Montgomery and Bucks counties.

In this role, you will be responsible for the coordination and supervision of all resident care provided within the agency.  Utilizes leadership skills to coordinate resident care services, serves as resident advocate, facilitates and ensures communication/collaboration among all agency team members.


The ideal candidate will meet the following requirements:

  • Current state license as Registered Nurse (RN)
  • Minimum of one year' geriatric health care and/or home health care experience
  • Current or eligible for certification in CPR
  • Must have reliable transportation
